Observation

Provider reported work in the home had began 2 weeks prior to the unannounced visit on 05/16/2022. Provider acknowleged to not having a gate that prevented the children the from entering an unlicensed space that had exposed outletts with live wires exposed and accessible to children. There are toxins that were exposed to the children such sa the paint and the sofas were standing on the side making it a tipping hazard for the children in care.
